ACR project for patients with diabetes (and/or other conditions)

The data is being processed for the purpose of delivery of a programme, sponsored by NHS Digital, to monitor urine for indications of chronic kidney disease (CKD) which is recommended to be undertaken annually for patients at risk of chronic kidney disease e.g., patients living with diabetes. The programme enables patients to test their kidney function from home. We will share your contact details with Healthy.io to enable them to contact you and send you a test kit. This will help identify patients at risk of kidney disease and help us agree any early interventions that can be put in place for the benefit of your care. Healthy.io will only use your data for the purposes of delivering their service to you. If you do not wish to receive a home test kit from Healthy.io we will continue to manage your care within the Practice. Healthy.io are required to hold data we send them in line with retention periods outlined in the Records Management code of Practice for Health and Social Care. Further information about this is available at: https://lp.healthy.io/minuteful_info/

TELL ME MORE ABOUT MY SUMMARY CARE RECORDS

When you visit an NHS or social care service, information about you and the care you receive is recorded and stored in a health and care record.

This is so people caring for you can make the best decisions about your care.

The information in your records can include your:

  • name, age and address
  • health conditions
  • treatments and medicines
  • allergies and past reactions to medicines
  • tests, scans and X-ray results
  • specialist care, such as maternity or mental health
  • lifestyle information, such as whether you smoke or drink
  • hospital admission and discharge information

National Coronavirus (COVID-19) Response: additional information will be included in your Summary Care Record (SCR) by default.

To help the NHS respond to the coronavirus (COVID-19) pandemic, The NHS has decided to include additional information in Summary Care Records for patients by default, with immediate effect. This is unless you have previously told us that you did not want your information to be shared. This will only apply for the duration of the coronavirus pandemic only. Unless you specify that you wish this information to be available after this emergency period, this change will be reversed.

Patients can be reassured that if they have previosuly opted-out of having a Summary Care Record ot declined to provide consent to share their additional inofmration, their preference will continue to be respected and applied.

If you wish to read more about this change or the additional information SCR in general, please click here.

CAN I MAKE A FREEDOM OF INFORMATION REQUEST?

The Freedom of Information Act obliges the practice to produce a publication scheme which is a guide to the ‘classes’ of information the practice intends to routinely make available.
