TELL ME MORE ABOUT MY SUMMARY CARE RECORDS

When you visit an NHS or social care service, information about you and the care you receive is recorded and stored in a health and care record.

This is so people caring for you can make the best decisions about your care.

The information in your records can include your:

  • name, age and address
  • health conditions
  • treatments and medicines
  • allergies and past reactions to medicines
  • tests, scans and X-ray results
  • specialist care, such as maternity or mental health
  • lifestyle information, such as whether you smoke or drink
  • hospital admission and discharge information

National Coronavirus (COVID-19) Response: additional information will be included in your Summary Care Record (SCR) by default.

To help the NHS respond to the coronavirus (COVID-19) pandemic, The NHS has decided to include additional information in Summary Care Records for patients by default, with immediate effect. This is unless you have previously told us that you did not want your information to be shared. This will only apply for the duration of the coronavirus pandemic only. Unless you specify that you wish this information to be available after this emergency period, this change will be reversed.

Patients can be reassured that if they have previosuly opted-out of having a Summary Care Record ot declined to provide consent to share their additional inofmration, their preference will continue to be respected and applied.
